Hiring a professional RN Care Manager

If you are an RN that takes client care seriously and want to have fun doing so, come join us! Help clients stay in their homes or transition to more appropriate settings. Work with our team of RN’s and MSW’s in coordinating care throughout King and Snohomish Counties.

Duties Include:

• Conduct Client Assessments
• Develop Client Care Plans
• Monitor Client Condition and Progress
• Nurse Delegation: Supervise and Train Caregivers
• Care Management: Coordination of Care and Resources
• Network within the Community

Must Have:

• A current and valid License to practice as a Registered Nurse in the State of Washington
• Graduation Credentials from an Accredited School of Nursing ; BSN preferred
• Two years of  Clinical Care experience required, home health experience or care management experience preferred
• Ability to read and write English proficiently, ability to communicate with Patients, Families and Personnel
• A current and valid Washington State Driver’s License and valid Vehicle Insurance
• Demonstrated Computer Skills  and writing skills to function properly in an office

Full time salaried position with benefits; shared on call duties for clinical issues with all clinical staff.
